Nurse Practitioner – Primary Care | Lead and Grow a Community Clinic

Heinen Wellness of Jennings is seeking a Nurse Practitioner to take an active leadership role in developing and growing a patient‑centered primary care clinic serving Jennings and the surrounding areas. This role offers a high level of autonomy, with the ability to manage day‑to‑day clinical operations, build your own patient panel, and shape the care experience from the ground up.

Our focus is on continuity of care, chronic disease management, and building meaningful, long‑term relationships with patients and families. This is not a high‑volume, quick‑turn environment. We are building a stable, relationship‑driven practice rooted in the community—and we are looking for a provider who wants to take ownership of that vision.

• Licensed Nurse Practitioner in the state of Louisiana
• Certification by AANP, ANCC, or equivalent
• Strong interest in outpatient primary care
• Comfort working independently and managing a patient panel
• A desire to take ownership of clinic operations and growth
• Commitment to building long‑term relationships with patients

• Manage and grow your own patient panel
• Perform comprehensive history and physical examinations
• Diagnose and manage acute and chronic conditions
• Develop and implement treatment plans
• Order and interpret diagnostic tests
• Provide patient education and preventive care
• Coordinate care and make appropriate referrals
• Maintain accurate and timely documentation
• Help shape clinic workflows and patient experience

• Up to $120,000 per year based on experience
• Up to 3% bonus based on clinic performance
• Increased compensation as your patient panel grows
