Physical Therapist to join our growing homecare team in Monmouth County
. In this role, you will be responsible for evaluating and providing skilled physical therapy services to patients in their homes, empowering them to live safely and independently while maximizing their mobility and quality of life.
- Deliver skilled, one-on-one physical therapy care in patients’ homes
- Partner with physicians and the interdisciplinary team to design effective care plans
- Promote independence and safety through personalized rehabilitation

What You Will Do:- Support the organization’s mission by striving for excellence and collaboration
- Assess and evaluate patients for physical therapy needs
- Develop, implement, and revise individualized treatment plans in coordination with physicians
- Document clinical status, changes, and outcomes
- Identify patient education needs and implement appropriate teaching strategies
- Monitor and maintain the quality and appropriateness of services provided
- Adhere to agency infection control and safety protocols
- NJ Physical Therapy license required
- 2–3 years of clinical experience preferred; homecare or community-based experience a plus
- Strong interpersonal and communication skills
- Excellent computer skills, including documentation via tablet or phone
- Valid NJ driver’s license and personal vehicle

Combination of indoor and outdoor work, driving, lifting, walking, standing, fine motor hand movement, and frequent use of phone and tablet for documentation.
In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $80,000.00 to USD $. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits.
