The Rose
F. Kennedy Center's Quality Improvement (QI) Coordinator is responsible for implementing and coordinating the Quality Management program of the Developmental Medicine Pediatric Division at Montefiore Medical Center. The QI Coordinator works closely with senior leadership to plan, organize, direct, and lead the personnel and work processes of the quality improvement program according to the standards of our Article 16 License with OPWDD.
The QI Coordinator uses data and tracking systems to assess, monitor and report program performance and determine ongoing improvement needs. The QI Coordinator also reports information pertaining to standards compliance, clinical performance, and patient experience to internal and external entities. The QI Coordinator chairs the quality improvement committee and assures execution of quality management initiatives throughout the Division.
- Ensure compliance with Article 16 regulations requiring dissemination and coordination of clinical services with non-Montefiore treatment providers, and prevent duplication of services between CERC/DOE/OMH and OPWDD settings.
- Document coordination efforts in EPIC for over 500 patients active on therapy with treatment plans.
- Participate in treatment/program planning and maintain records, statistical reports, and other documentation to ensure appropriate treatment activity and program efficacy.
- Use data to assess, monitor, and report program performance and identify ongoing improvement needs.
- Report standards compliance, clinical performance, and patient experience to internal and external stakeholders.
- Chair the quality improvement committee and oversee execution of quality management initiatives across the Division.
- Build transparent partnerships with community partners and coordinate care activities.
- Initiate and problem‑solve utilizing available resources within the scope of responsibility.
- Maintain knowledge of interdisciplinary treatment settings and Article 16 regulations.
- Minimum 3 years of work‑related experience.
- Bachelor's Degree required.
- Training required: OPWDD CCO training, Medisked training.
- Extensive knowledge of Article 16 regulations and care coordination.
- Strong understanding of community partners and ability to build transparent partnerships.
- Initiative and problem‑solving skills.
