- High school diploma or G.E.D. certificate.
- Access to reliable transportation.
- Minimum of six (6) months work experience in a supervised setting, preferably health care facility.
- Demonstrated ability to read, write, and follow a written Plan of Care/Aide Care Plan.
- Good verbal, written, and interpersonal skills.
- Previous experience as an at‑home caregiver or in senior communities.
- Proficient in speaking both English and Spanish.
- Males encouraged to apply.
- New Jersey Home Health Aide Certification required.
- Assist with daily activities to help clients stay independent and in their own homes.
- Provide assistance with personal care activities including but not limited to: bathing, shaving, skin/nail care, shampooing, oral hygiene, dressing, range of motion exercises, and assistance in ambulation exercises.
- Assist with mobility, transfers and range of motion exercises.
- Handle meal planning, preparation and feeding.
- Properly manage household needs.
- Provide companionship and respite services for the family.
- Assist in the administration of medications that are ordinarily self‑administered under the direction and supervision of the Registered Nurse.
- Reports any observed or reported changes in the client’s condition and/or needs to the Registered Nurse. Follows reporting guidelines in the Plan of Care/Aide Care Plan.
- Documents care provided and completes the forms required for the client’s records. Completes the appropriate records to document care given and pertinent observations.
- Promotes personal safety and a safe environment for clients by observing infection control practices, following Agency guidelines, and reporting unsafe situations to the supervisor.
- Demonstrates safe practice in the use of equipment.
- Communicates effectively with family and all members of the interdisciplinary team.
- Maintains confidentiality in all aspects of the job.
