Purpose
The nurse practitioner helps with all aspects of patient care, including diagnosis, treatments and consultations. Performing independently or as part of a treatment team in educating patients about preventative care and the prescribed treatments. Will also conduct physicals, order tests and serve as a patient's primary healthcare provider and prescribe medications.
Duties and Responsibilities- Follows all PCMH procedure and policies.
- Provide general and preventative care, conduct check‑ups, treat illnesses, order lab tests, and prescribe medication for children and adults.
- Performs comprehensive history, physical, and psychosocial assessment of patients attending the clinic.
- Diagnoses and treats common acute conditions, illnesses or minor trauma within legally accepted protocols in collaboration with the physician.
- Develops a treatment plan for every patient.
- Assesses the client's psychosocial, emotional, physiologic and environmental needs and priorities.
- Informs, teaches, counsels, and guides the patients to assume responsibility for the management of their own health care.
- Schedules appointments for clients and makes arrangements for consultations and referrals as needed. Evaluates the client's response to medical treatment and revises the plan of care as appropriate.
- Collaborates with multidisciplinary team members by making appropriate referrals.
- Participates in grand rounds and case conferences.
- Acts as a role model and resource person for those who are working directly and indirectly with the clients.
- Other clinical and/or administrative duties and responsibilities may be assigned at the discretion of the Medical Provider or Chief Executive Officer.
- Master’s or Doctorate degree in Nursing from an accredited program.
- Certification as a Pediatric Nurse Practitioner (PNP) – must be licensed for New Jersey.
- Nurse Practitioner licensure, Current DEA registration and CDS.
- Strong clinical assessment and diagnostic skills.
- Excellent communication and patient education abilities.
- Commitment to compassionate, patient‑centered pediatric care.
- Two years related experience (preferred).
- American Board of Pediatric Nurse Practitioner Certificate and master’s in nursing and additional diploma within the scope of Pediatric Nurse Practitioner.
