Coordinator of Care RN; Community Health Nurse - Home Care CHHA

MJHS Home Care is a unit of a large not‑for‑profit health system that delivers advanced clinical services and emotional support to patients recovering from illness, injury, or surgery in their homes.

• Coordinate, implement, and continuously evaluate the home‑care needs of patients.
• Review patient insurance information and collaborate with physicians to develop and update comprehensive, interdisciplinary care plans.
• Ensure pertinent findings are shared with families and professional caregivers.
• Case‑manage the care and services for assigned patients, aiming for positive clinical outcomes and reduced rehospitalization risks.
• Provide patient assessment, medication management, and patient/family education.
• Demonstrate expert knowledge unique to community health nursing and solve problems independently while interacting with an integrated team.
• Support the agency’s established financial goals through efficient resource utilization.

• Current NYS RN license and registration.
• Bachelor’s degree in nursing preferred.
• Minimum of one year acute medical‑surgical nursing experience.
• CHHA (Certified Home Health Aide) experience preferred.
• Driver’s license and reliable vehicle required.
• Excellent communication, documentation, and time‑management skills.
• Excellent clinical assessment ability.
• Strong ability to solve problems independently and work collaboratively with an integrated team.
