NJ Palliative Care Solutions partners with leading health systems, including RWJBarnabas Health, to deliver palliative care across New Jersey. We are hiring a Nurse Practitioner or Physician Assistant to provide hospital‑based palliative care at Jersey City Medical Center for patients living with serious illness. Palliative care experience is required for this inpatient role.

- Perform at the top of your license, providing best practice, holistic, patient‑centered, evidence‑based interdisciplinary palliative medicine.
- Provide palliative care consultations and follow‑up visits across the hospital continuum – from high‑acuity ED and ICU encounters to ongoing management on medical, surgical, and specialty units.
- Manage complex symptoms for patients with advanced illness and chronic disease.
- Lead goals‑of‑care and advance care planning conversations with patients and families, coordinating with partner SNFs and/or hospices when appropriate.
- Partner with the interdisciplinary team and hospital teams, fully integrating into hospital workflows and shaping the palliative medicine practice.
- Complete timely documentation aligned with clinical, billing, and quality standards.
- Spend as much time on discharge planning as on intake, managing transitions from inpatient to the patient’s next setting, considering each patient’s longitudinal preference and best interests.
- Active New Jersey Nurse Practitioner or Physician Assistant license and national board certification.
- DEA + CDS registration, or ability to obtain by start date.
- 2+ years of palliative care experience required.
- Palliative care certification (e.g., ACHPN / CHPN) preferred.
- Strong communication, clinical judgment, compassion, and comfort working on interdisciplinary teams.
- Competitive base salary: $130,000–$160,000 per year.
- Annual performance bonus up to 12.5% of base salary, rewarding both productivity (RVUs) and quality of care.
