Mission Statement

The Division of Vascular Surgery & Otolaryngology-Head and Neck Surgery at Rutgers NJMS is the regional leader in providing quality clinical care to patients with vascular and otolaryngologic diseases, educating future vascular surgeons and otolaryngologists, and advancing care through clinical and translational research.

Summary

APN will be an integral member of the Vascular Surgery & Otolaryngology Teams at Rutgers NJMS, providing care to patients, improving efficiency of care, and participating in both quality improvement and clinical research programs.

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Provide patient-centered care and create a friendly, welcoming, and safe environment.
  • Communicate clearly to ensure understanding and resolve complaints or concerns immediately whenever possible.
  • Collaborate with the multidisciplinary team to coordinate patient care plans.
  • Exceed patient/family expectations for service and act as a role model for Service Excellence.
  • Maintain professional composure and confidence in the care setting.
  • Achieve customer service scores that meet or exceed targets.
  • Respect patient rights and acknowledge the uniqueness of each patient.
  • Collect comprehensive patient health data/history, including medical, surgical, family and social history.
  • Collect, interpret and document diagnostic tests and procedures in the Electronic Medical Record.
  • Complete medication reconciliation for each visit.
  • Perform a comprehensive physical assessment including review of systems, evaluation of manifestations, toxicities, and anticoagulation management.
  • Evaluate patients in the Emergency Department and for inpatient consultations and assist in management.
  • Analyze assessment data with physicians to determine diagnosis and develop a comprehensive plan of care.
  • Evaluate laboratory studies and diagnostic tests, providing appropriate interventions.
  • Monitor patient progress toward treatment outcomes and watch for disease progression/recurrence.
  • Manage medical and anticoagulation emergencies per protocol/policy.
  • Prescribe and/or order medications.
  • Obtain surgical and procedural consents.
  • Ensure completion of all required pre-operative tests, forms and documents.
  • Document assessments, plans, modifications, and evaluations in the electronic medical record.
  • Triage patient problems via telephone calls.
  • Coordinate continuity of care between departments/services and collaborate with Clinical Care Coordinators, Patient Navigators and medical staff.
  • Prepare discharge packets with needed prescriptions and follow-up office visits.
  • Perform daily rounds with physicians, completing clinical tasks such as progress notes, orders, dressing changes.
  • Insert and remove central venous catheters, drains, arterial lines and sheaths; provide tracheostomy and gastrostomy care.
  • Perform flexible laryngoscopy, nasal endoscopy, and incision & drainage procedures.
  • Assist during outpatient bedside surgical procedures.
  • Coordinate patient care teams.
  • Dictate discharge summaries and interim update letters to referring physicians.
  • Assist in bed management, arranging admissions and transfers between care levels.
  • Interface with families to provide information on medical care and progress.
  • Monitor medication side effects and provide appropriate interventions.
  • Provide health promotion and disease/illness/injury prevention.
  • Offer patient and family counseling and reassurance for psychological or psychosocial issues.
  • Educate patients, families and caregivers on disease etiology, treatment adherence, supportive care, follow-up, potential adverse effects, costs, risks, benefits of alternative therapies.
