Schedule: Full-Time
Salary: $90,000 - $100,000 annually

Optima Care Harborview is seeking a compassionate, experienced, and dedicated Social Services Director to lead our Social Services Department. In this leadership role, you will ensure residents and their families receive exceptional emotional, psychosocial, and practical support while promoting the highest quality of care throughout their stay.

• Direct and oversee all functions of the Social Services Department.

• Supervise, mentor, and support social services staff while promoting high-quality, resident-centered care.

• Coordinate discharge planning, community resource referrals, financial assistance programs, and post-discharge services.

• Conduct psychosocial assessments and develop individualized care plans in collaboration with the interdisciplinary team.

• Ensure behavioral health and psychosocial services are integrated into each resident's care plan.

• Educate residents and families regarding resident rights, advance directives, and available support services.

• Participate in resident care conferences, family meetings, and resident/family council meetings.

• Maintain current knowledge of all federal, state, and local regulations governing skilled nursing facilities and recommend policy updates when appropriate.

• Assist residents with limited English proficiency by facilitating communication and accessing interpreter services when needed.

• Maintain accurate, timely, and compliant documentation in accordance with facility policies and regulatory standards.

• Build and maintain strong relationships with hospitals, community agencies, and outside service providers.

• Assist during state surveys, audits, and regulatory inspections while ensuring departmental compliance.

• Foster a positive, compassionate environment that supports resident dignity, independence, and quality of life.

• Certified Social Worker (CSW) or Master of Social Work (MSW) required.

• Minimum of two (2) years of supervised social work experience in a healthcare setting.

• Previous skilled nursing, rehabilitation, long-term care, or nursing home experience strongly preferred.

• Prior supervisory or leadership experience preferred.

• Thorough knowledge of federal and state regulations related to skilled nursing facilities.

• Excellent leadership, organizational, interpersonal, and problem-solving skills.

• Strong documentation and communication skills.

• Ability to effectively manage multiple priorities in a fast-paced environment.

• Compassionate, patient-centered approach with the ability to work effectively with diverse populations.

• Must meet all pre-employment health screening and background check requirements.
