Crisis Intervention Case Manager
Location:
Jersey City, NJ
Mobile Response & Stabilization Services
The Mobile Response program provides counseling, guidance, and referral services to address juvenile crises and helps stabilize and preserve the family unit. Services are available 24 hours a day, seven days a week, 365 days a year, responding to calls within one hour of referral.
Qualifications- Bachelor's Degree in Social Work or a related field
- Valid driver’s license
- Bilingual fluency in English and Spanish (preferred)
- At least one year of experience working with children and families in crisis assessment and intervention (preferred)
Annual Salary: $43,500 - $46,500
Responsibilities- Provide in-home and community-based crisis de‑escalation/intervention and assessment, safety planning, and stabilization and case management services to families and children according to individualized service needs.
- Coordinate discharge/transition services including referrals and linkages to appropriate services and supports based on the level of need of the child/family.
- Respond to calls within 1 hour, as required by contract, serving families 24/7/365 with emergency assistance.
