Jesup, GA: Nurse Practitioner or Physician Assistant, PRN

About the Role

Advanced Practice Provider for skilled nursing facilities within a close‑knit mission‑driven care community.

• Provide high‑quality, patient‑centered care to residents in skilled nursing facilities.
• Conduct daily rounding, prescribe medications and treatment plans, and address acute/emergent issues.
• Communicate with facility nursing staff via phone or secure messaging as needed.
• Maintain reliable on‑site attendance for scheduled shifts.
• Ensure compliance with clinical standards, facility policies, and regulatory requirements.

• Hold an active U.S. license and be board‑certified by ANCC, AANPCB, PNCB or AACN (for Nurse Practitioners) or by NCCPA (for Physician Assistants).
• Specialties may include Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.
