Licensed Practical Nurse (LPN) - Adult Day Care

Susquehanna Nursing & Rehabilitation Center/Golden Days Adult Day Healthcare Program is looking to add a Licensed Practical Nurse (LPN) to its Adult Day Care team. In this role, you'll provide nursing care and support to registrants while helping ensure a safe, engaging, and compassionate environment. Rate: $29/hour

Shift: Part-Time

Schedule:

Mondays and Tuesday, 8:30 am- 4:30 pm State Holidays Off

Responsibilities:

  • Provide nursing care to registrants in accordance with established policies and procedures
  • Supervise nurse aides in providing care, meals, and activities
  • Monitor registrant health and well-being and document care accurately and timely
  • Communicate changes in condition to responsible parties and maintain appropriate documentation
  • Assist with coordinating transportation for registrants as needed
  • Promote a safe, supportive, and person-centered environment
  • Collaborate with interdisciplinary team members to support quality care
  • Perform additional duties as assigned

Requirements:

  • Current NYS Licensed Practical Nurse (LPN) license required
  • Strong communication and interpersonal skills
  • Ability to supervise and support nursing assistants and other staff
  • Strong organizational and documentation skills
  • Ability to provide compassionate, resident-centered care
  • Professional, dependable, and able to work effectively as part of a team
