Responsible and accountable for all elements of the nursing process when providing and or supervisor direct patient care. Assumes responsibility for the coordination of care focused on patient education, self management, and customer satisfaction throughout the continuum of care. Influences care outcomes by collaborating with members of the interdisciplinary team. Elements of performance include active participation in the program quality initiatives as well as customer service programs.

Responsible for providing component evidence based care to assign patients provides patient management care, coordination, and disposition planning for veterans included, but not limited to addressing mammography, cervical cancer screening, and or bone density testing. Assist veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex healthcare needs. With multiple providers across all levels and sites of care.

Coordination also includes assuring services interface with the veterans affairs and community programs to provide continuity of care to veterans. Provides oversight of the patient's journey through diagnostic and treatment continuum, ensuring that all steps in the process or schedule, timely and completed while tracking the plan of care. Coordinates and facilitates as needed women's health care, including contacting the veteran, facilitating appointments, and authorizations, track and receipt of results and other medical documentation, and notifying providers of needed follow up care.

Will interface with outside referral agencies, and ensure that their business and clinical practices follow regulatory standards. Anticipate and implements preventative patient care measures. Provides guidance and education to parents and caregivers and collaborate with other professionals at all levels. Addresses psychosocial as well as nursing and medical needs of patience and their family/caregivers through participation with the interdisciplinary care team.

Ensures the plan of care and educational requirements are individualized and tailored specifically to support each individual patient needs. Reports status of service needs and contributes to problem resolution and modification of processes as necessary. Supports the woman's health program and evaluate care and outcomes to ensure timely and appropriate revision of gender specific services. Advocates for holistic care through patient/family education and provider interaction and implements and educational plan to meet the patient care needs.

Demonstrates leadership and delivering and improvement women's healthcare through collaborative strategies with others. Evaluate practice in an ongoing process, based on best evidence. Provides peers with informal constructive feedback for improvement. Fosters a safe and supportive environment conducive to the professional development of healthcare professionals. Contributes, professional, nursing and perspective and discussion with the interdisciplinary team. Partners with others to affect change and produce optimal outcomes.

Supports colleagues through knowledge sharing to provide safe, quality, nursing care. Shares, educational findings, experiences, and idea with peers. Questions clinical practices for the purpose of providing evidence based care. Participates and activities and strategies to sustain and evidence based practice culture.
