We are looking for an RN to join our Home Health team and make a difference in the Clarksville, TN area and surrounding communities.

Position Overview

The RN provides and directs nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies.

Responsibilities
  • Create, implement, execute, and coordinate the overall plan of care to promote wellness and prevent re-hospitalization for patients in their homes.
  • Maintain continuity of patient care by planning and exchanging information with the physician, agency personnel, patient, family, and community resources.
  • Make the initial and/or comprehensive nursing evaluation visit, determine the primary focus of care, develop the plan of care within state specific guidelines, and ensure patients meet home health eligibility and medical necessity guidelines as defined by the payer.
  • Maintain and submit accurate documentation and clinical notes, including the patient’s condition, appropriate goals, interventions, and the patient’s response to home health care.
  • Establish and maintain an effective and efficient visit route.
  • Provide high quality clinical services within the scope of practice as defined by the state laws governing the practice of nursing, in accordance with the plan of care, from admit through discharge.
Qualifications / Must Haves
  • One (1) year of clinical RN experience.
  • Current RN license in the state where services are rendered.
  • Current CPR certification.
  • Driver’s License, vehicle insurance, and access to a dependable vehicle or public transportation.
  • Must be able to reliably commute to multiple patient residences daily within the geographic area assigned.
