Case Manager Registered Nurse

Job Summary

Under the supervision of the Nurse Manager, the Case Manager performs the primary function of an RN in assessing, planning, implementing, and evaluating the care of patients in the department.

• Utilize good problem‑solving abilities, ensuring problems are resolved and calling on other knowledgeable parties as necessary.
• Evaluate collected data and prepare reports assessing accuracy, completeness, timeliness, progress, adverse trends, and appropriate recommendations or conclusions.
• Maintain confidentiality by following hospital rules and reminding others when necessary (HIPAA).
• Actively participate in continuous quality improvement activities by making suggestions for improvement and helping to implement changes that support quality control, quality assurance, safety, and inspection control practices.
• Participate in the team spirit of the department, helping to accomplish the department mission and lending support to other members of the team as appropriate.

• Graduate of an accredited RN program.
• Registered Nurse (RN) license required or RN Compact State Licensure in the State of Residence.
• Case Management Certification (CCM) or American Case Management Association (ACMA) certification preferred.
• 3‑5 years acute‑care experience with a preference for Case Management or Utilization Management.
