Gentiva is seeking a compassionate Hospice RN / RN Case Manager to lead in-home patient care. You will perform assessments, deliver nursing interventions, and develop plans of care in collaboration with the IDG.
You will educate patients and families, coordinate with physicians and social workers, and ensure accurate documentation and compliance with hospice standards. Experience in hospice or palliative care is preferred; an active RN license, CPR, and a valid driver's license are required.
