The SSVF program assists veterans who are homeless or at‑risk of homelessness to end their housing crisis. The Housing Case Manager will: assist veterans in ending their crisis; enhance independent living skills and provide supportive services; connect them with community resources; empower them to maintain long‑term housing stability and self‑sufficiency.

Responsibilities include navigating the vulnerabilities of homeless veterans, building relationships with landlords, advocating for participants, and helping obtain legal aid to remove barriers to housing. The role promotes housing justice, provides culturally inclusive trauma‑informed services, and advocates for participants.

Qualifications:

• Bachelor’s degree in social work or related field with less than five (5) years of experience, or five (5) years of experience without a degree, or a veteran with three (3) years of related work experience.
• Knowledge and ability in family needs assessment, resource access, advocacy, counseling, and collaboration with local service providers. Commitment to project principles.
• Excellent verbal and written communication, organizational skills, and creative problem‑solving.
• Valid driver’s license, personal automobile, liability insurance, and willingness to travel up to 70% of the time within service counties.
• Required completion of case management and Critical Time Intervention (CTI) training within 90 days of hire.

• Build partnerships with community partners, service providers, and government agencies to address structural challenges for people of color experiencing homelessness.
• Work sensitively with veteran families and diverse cultural/ethnic backgrounds to meet individual housing needs.
• Actively remove barriers to housing for veterans, especially people of color, addressing economic mobility, affordable housing, criminal justice, behavioral health, and family stabilization.
• Explain HUD‑VASH and Section 8 programs; advise participants on eligibility.
• Advocate for veterans and act as liaison with landlords, rental agencies, community organizations, and other service agencies.
• Read, interpret, and explain rental contracts, ensuring fairness and enforce ability.
• Perform assessments, develop and monitor case plans, and conduct follow‑up activities.
• Establish linkages with agencies and service providers; provide referrals and coordination with VAMCs and other providers.
• Develop individualized housing search plans and set goals with participants.
