Provide high-quality rehabilitative care to residents in a skilled nursing facility, supporting mobility, function, and overall quality of life.
Responsibilities- Evaluate, plan, and implement individualized treatment programs
- Collaborate with the care team for comprehensive patient care
- Document patient progress and adjust treatment plans as needed
- Educate patients and families on therapy goals and exercises
- Current PT license
- Minimum 1 year of PT experience (SNF experience preferred)
- Current BLS certification
- Strong clinical skills and ability to work independently
