As an MDS Coordinator, you will play a crucial role in ensuring the accuracy and completeness of our patients' Minimum Data Set (MDS) assessments, while providing exceptional support to our healthcare professionals. Responsibilities:

• Coordinate the collection and outcome of MDS assessments for all residents, ensuring compliance with Medicare guidelines and regulatory requirements
• Audit and verify the accuracy of MDS data, making necessary corrections and updates in a timely manner
• Collaborate with healthcare professionals to obtain necessary information and complete MDS assessments
• Provide education and support to clinical staff on MDS assessment processes and guidelines
• Participate in quality improvement initiatives to enhance the accuracy and completeness of MDS assessments
• Develop and maintain organized systems for tracking and storing MDS documentation

Requirements:

• Minimum 2 years of experience in MDS coordination or a related field
• RN License is required.
• Strong analytical and problem-solving skills
• Excellent communication and interpersonal skills
• Ability to work in a fast-paced environment with multiple priorities
• Familiarity with MDS software and electronic health records (EHRs)
