As a Certified NP, you ll serve as a key clinical provider in skilled nursing facilities, delivering high-quality care to patients while collaborating with an interdisciplinary healthcare team.

Your responsibilities include:

• Performing comprehensive physical exams and reviewing medical histories
• Ordering and interpreting diagnostic tests, including laboratory work and imaging
• Diagnosing and managing acute and chronic medical conditions
• Prescribing medications and developing individualized treatment plans
• Collaborating with physicians, nurses, therapists, and facility staff
• Educating patients and families on health conditions, treatment plans, and preventive care
• Documenting patient encounters accurately and efficiently in the electronic medical record

We welcome both experienced providers and motivated new graduates.

• Current NP (or eligibility to obtain)
• Current DEA registration or willingness to obtain prior to start date
• New graduates encouraged to apply—training and mentorship available
• Interest in geriatrics, internal medicine, family medicine, or post-acute care
• Excellent communication and teamwork skills
• Self-motivated with a patient-centered approach
