Conemaugh Physician Group (CPG) Somerset Primary Care is a practice that cares for the health of an individual and their family through all stages of life, offering regular wellness visits, screenings, acute illness visits and diagnosis, management of chronic conditions and guidance for proper health and promoting a healthy lifestyle. We coordinate referrals for expertise of certain illness or injuries and ensure specialized care and treatment for each patient.

This position will be working in the clinical setting rooming patients, doing vital signs and preliminary tasks such as assisting with questionnaires, etc. You will be responsible to do immunizations if needed, assist the providers with treatments, phone triage between the patient and the provider, prescription refilling and quality measures.

A Licensed Practical Nurse (LPN) who excels in this role:

  • Assists medical staff in carrying out plans for care of patients.
  • Determines nursing care needs of the patient.
  • Maintains patient flow and helps to keep daily schedule flowing smoothly.
  • Triage patient calls and communicates with provider patient needs.
  • Anticipates both patient and providers needs by making sure lab tests and reports are available for the appointment for review.
  • Calls patients results and scans to orders in EMR.
  • Coordinates special activities with the hospital, i.e. direct admissions from the office or any other unique requirements.
  • Handles patient telephone inquiries.
  • Assists physician and mid-level provider in completing prescription renewals electronically.
  • Documents in the electronic medical record patient vitals, updates med lists, assists in completing electronic flow sheets and documents electronically all treatments, injections etc
  • Transcribes lab and other medical data to charts, as needed
  • Performs special tests, i.e. audiometry, eye tests, and EKG's.
  • Removes sutures.
  • Inventory and order medical supplies, as needed.
  • Monitor and works their In-Basket daily.
  • Travel to satellite locations as needed.

Applicants should have a current state LPN license and graduate of an of a program for licensed practical nurses. Additional requirements include:

  • One year in a physician office setting.
  • Electronic medical record experience either in acute care setting or medical office practice preferred
