RESPONSIBILITIES:
- As a Registered Nurse Assessment Coordinator (RNAC), your primary responsibility is determining the appropriate level of care for our residents.
- Scheduling and overseeing all resident MDS Assessments.
- Participating in the pre-admission process to determine potential residents’ level of care.
- Reviewing all MDS assessments and related forms to certify timely completion.
- Participate in the Quality Assessment and Assurance Program.
- Understand and follow Resident Care Plans.
- Ensure accurate documentation during your shift.
- Complete accurate charting, reporting, and progress notes.
REQUIREMENTS:
- Possess an active and good-standing RN License.
- Long term care experience preferred.
- Experience as an RNAC preferred.
