Aging Care Manager
The Aging Care Manager position offers a rewarding opportunity to make a meaningful difference in the lives of older adults. This position provides comprehensive assessment, care coordination, advocacy, and supportive services to older adults who may be unable to fully meet their own needs. The Aging Care Manager works to ensure individuals receive the appropriate resources and services necessary to maintain their independence, dignity, safety, and quality of life while remaining in their homes and communities whenever possible.

This role focuses on investigating concerns involving older adults and coordinating services that promote their safety, stability, and well-being. As an Aging Care Manager 3, you will perform the following duties:
- Case Investigations: Conduct investigations of alleged abuse, exploitation, or neglect
- Risk Assessment: Evaluate medical, social, and psychological needs to determine risk levels
- Protective Actions: Implement measures that safeguard vulnerable older adults
- Legal Coordination: Recommend court interventions, including guardian ships and involuntary commitments
- Service Documentation: Document all actions, findings, and service plans
- Agency Collaboration: Coordinate plans with partnering agencies to refer individuals for additional support
Work Schedule and Additional Information:
- Full-time employment
- Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with 30-minute lunch.
- Telework: You will not have the option to telework in this position.
- Salary: In some cases, the starting salary may be non-negotiable.

Minimum Experience and Training Requirements:
- One year as an Aging Care Manager 2 or Aging Program Assessor; or
- One and one-half years of professional human services, law enforcement, or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
- An equivalent combination of experience and training which includes six months of professional human services, law enforcement, or nursing experience.

- Hiring preference for this vacancy may be given to candidates who live within Cambria County. If no eligible candidates who live within Cambria County apply for this position, candidates who reside in other counties may be considered.
- You must be able to perform essential job functions.
Legal Requirements:
- A conditional offer of employment will require a medical exam and a drug screening.
- This position falls under the provisions of the Child Protective Services Law. Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
