Department: 37650 Advocate Lutheran General Hospital - Inpatient Rehabilitation

Schedule Details:
One weekend day every month, one holiday, and one weekend day around the holiday

Major Responsibilities
  • Interprets physician referrals and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations using appropriate methods.
  • Establishes a plan of care and implements individualized treatment based on evaluation results, functional goals, and appropriate techniques.
  • Monitors and evaluates outcomes, adjusting the individualized treatment plan as needed and providing status updates to physicians and teammates.
  • Identifies factors affecting patient physical and psychosocial comfort and attends to these needs.
  • Plans and completes patient discharge, including written home programs, patient/family teaching, referral for community services, transition planning and equipment ordering.
  • Communicates and collaborates with stakeholders to achieve optimum patient care.
  • Completes all required documentation per department policy and accreditation standards, including patient evaluations, progress notes, team conference reports, and discharge summaries.
  • Records patient care and progress for each assigned patient.
  • Completes daily statistics, billing, and scheduling.
  • Applies insurance guidelines regarding authorization and number of visits.
  • Maintains site/department productivity standards.
  • Adheres to all billing and documentation policies, procedures, and timelines.
  • Cleans up after patient treatment, puts away equipment, and keeps the area neat and orderly.
  • Performs manual techniques, patient positioning, transferring, and transporting duties that require lifting, pushing, and pulling.
  • Demonstrates skill in using modalities, exercise equipment, and other devices per department orientation checklist.
  • Applies knowledge of indications and contraindications of modalities and exercise.
  • Promotes patient safety by participating in safety initiatives and following all safety and infection control measures.
  • Participates in performance improvement activities to resolve problems affecting patient care outcomes and improve quality of care.
  • Manages assigned caseload, informs leadership of coverage needs, and assists with coverage based on department and regional needs.
  • Pursues professional growth through continuing education, department in‑services, and literature review.
  • Orients new staff to clinical and department policies as requested.
  • Participates in co‑worker evaluations by providing verbal or written feedback as requested.
  • May supervise physical therapy assistants, delegating patients, providing direction, recommendations, and ongoing supervision while maintaining responsibility for patient plans of care and goals.
  • May supervise physical therapy extenders (rehab aides/techs) in administering treatments and teaches them appropriate techniques.
  • May supervise physical therapy students after completing one year of clinical experience.
Minimum

Job Requirements Education
  • Doctorate degree in Physical Therapy from an accredited school or educational requirements commensurate with the date of graduation.
Certification / Registration / License
  • Licensed Physical Therapist in the state where you will work.
  • Basic Life Support (BLS) for Healthcare Providers obtained within 90 days unless waived by the department leader.

  • Demonstrated organization, decision‑making, and problem‑solving skills.
  • Successful completion of all competency assessments and requirements for patient care as per site standards.
  • Knowledge of principles of growth and development over the lifespan.
  • Ability to assess patient data, interpret information, and identify age‑specific care needs.
Physical Requirements and Working Conditions
  • Must sit, stand, walk, lift, and squat throughout the workday.
  • Lift up to 50 lbs. from floor to waist.
