Ardor Health Solutions is seeking a Physical Therapist Assistant for a travel SNF/Rehab contract at Centralia Manor in Centralia, Illinois

Centralia Manor is a skilled nursing facility located on the Liberty Village of Centralia campus. The facility provides skilled nursing and rehabilitation, including its Bounce Back Rehabilitation program, which focuses on restoring strength and improving daily function to help patients recover and return to greater independence.

, providing a substantial adult and geriatric rehabilitation environment.

The PTA will work under the direction of the Physical Therapist to carry out established plans of care for patients requiring short-term rehabilitation and ongoing skilled therapy.

Responsibilities may include:

• Therapeutic exercise and strengthening
• Gait and balance training
• Transfer and functional mobility training
• Range-of-motion activities
• Fall-prevention and safety training
• Patient and caregiver education
• Monitoring and documenting patient progress
• Collaborating with the supervising PT and interdisciplinary rehab team
