Quality Improvement RN – Lightways Hospice and Serious Illness Care (Joliet, IL)

Founded in 1982 as Joliet Area Community Hospice, Lightways Hospice and Serious Illness is an independent, non‑profit healthcare provider licensed in 11 counties in northwest Illinois. We have a state‑of‑the‑art facility and the first free‑standing inpatient Hospice Home in Illinois. We have a 35‑year history for providing compassionate professional care to terminally ill patients and their families. We are state licensed and Medicare/Medicaid certified.

We are currently seeking a full‑time Quality Improvement RN to join our Quality, Compliance, and Risk Management team. This role will co‑lead QAPI initiatives, serve as the clinical lead for CHAP accreditation readiness, and collaborate on policy development and process improvement.

• Co‑lead QAPI initiatives.
• Clinical lead for CHAP accreditation readiness.
• Collaborate on policy development and process improvement.
• Educate clinical staff on documentation standards and infection control protocols.
• Conduct audits and support regulatory compliance.

• Illinois Registered Nurse with a minimum of two‑year experience in home health or nursing home/long‑term care.
• Previous Hospice experience helpful.

Work schedule:

Monday – Friday, 8:00am to 4:30pm.
