In-Home Traveler Nurse Practitioner or Physician Assistant - Illinois

Full Time Clinician (Nurse Practitioner or Physician Assistant)

Signify Health, part of CVS Health®, seeks a clinician to perform in‑home health evaluations. You’ll travel up to 100% of the time across a defined coverage area, which may include overnight hotel stays, car rentals, and flight requirements. Holding multi‑state licenses will be required.

The visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and, if ordered by the health plan, additional diagnostic tests (diabetic eye exams, spirometry, etc.). You will use an iPad and an intuitive clinical workflow. You are not required to prescribe medicine, order lab tests or alter treatment regimens.

• Deliver high‑quality in‑home evaluations and provide connections to needed care.
• Travel locally and within licensed states.
• Conduct virtual visits as needed.
• Perform or order diagnostic and preventive services (spirometry, DEE, bone density screening, etc.).

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Ability to obtain and maintain multi‑state licenses as required.
• Coverage of all counties within state and ability to flex to other business areas.

40 hours (Full‑time)

$95,738.00 – $ (base hourly rate or annual full‑time salary, depending on role, will vary based on experience, education, geography, and other factors).
