Patient Services LPN

Center Well Home Health, part of Humana Inc., is seeking a Patient Services LPN to deliver compassionate in-home care to seniors and adults with chronic conditions. In this role, you'll provide direct nursing care, perform assessments, administer medications, and support recovery and disease management in the home setting. You'll collaborate with an interdisciplinary team, follow individualized care plans, and use electronic tools to document visits.

Responsibilities

• Provide in-home nursing care, including assessments, wound care, and medication administration for senior and adult patients
• Monitor patient conditions, document clinical findings, and communicate changes to the interdisciplinary care team
• Develop and follow care plans in coordination with RNs, therapists, and physicians
• Educate patients and caregivers on disease management, medications, and safety in the home
• Use electronic health records and mobile tools to document visits accurately and timely
• Support patients in maintaining independence and preventing hospital readmissions
• Ensure compliance with clinical protocols, safety standards, and regulatory requirements
• Participate in team meetings and case conferences to coordinate patient-centered care
