Dialysis Therapies Registered Nurse, RN

• Deliver high-quality, patient-centered care for a new home dialysis program in the Chicago metropolitan area
• Educate, train, and support patients and caregivers performing home hemodialysis/peritoneal dialysis
• Travel daily within approximately a 30-mile radius across the Chicago metropolitan area for patient training and home assessments
• Train patients and caregivers on equipment setup, treatment initiation, monitoring, and troubleshooting
• Provide education on infection control, vascular access care, and emergency protocols
• Conduct initial patient assessments, medical history reviews, and home environment evaluations
• Develop and implement individualized care plans with the interdisciplinary team
• Provide routine training, education, and guidance to patients and caregivers
• Monitor patient progress through follow-ups and assessments and adjust care plans
• Maintain accurate and timely documentation in compliance with state and federal regulations and CMS guidelines
• Ensure care aligns with organizational policies and quality standards
• Train patients on dialysis equipment maintenance and use
• Educate, train, and help coordinate delivery and inventory management of supplies
• Serve as a patient and caregiver resource and advocate
• Facilitate communication among patients, physicians, and healthcare providers
• Provide ongoing education for managing hypotension, bleeding, and equipment malfunction
• Participate in Quality Assurance & Performance Improvement or CQI
• Record arrival and departure times via ADP
• Promote and follow company mission, vision, and core values
• Receive and act on feedback; report incidents; check and respond to work email
• Maintain regular attendance and punctuality and perform other assigned duties

• Graduate of an accredited School of Nursing
• Current appropriate state licensure
• Minimum of 12 months current experience in hemodialysis, including home hemodialysis
• Experience in Peritoneal Dialysis preferred, but not required
• Current or successful completion of CPR BLS Certification
• Must meet the practice requirements in the state in which he or she is employed
• Must meet the state or facility health mandates, if applicable
• Strong knowledge of dialysis operating requirements
• Strong clinical assessment and critical thinking skills
• Excellent communication, teaching, and interpersonal skills
• Ability to work independently and manage multiple patients in a remote setting
• Proficiency with electronic medical records (EMRs) and Microsoft Office Suite
• Ability to travel to patients' homes across the assigned state
• Ability to work remotely and in an office setting when required
• Valid driver's license and reliable transportation
• Ability to lift and move dialysis equipment and supplies, as required
• Ability to be flexible and adhere to innovative scheduling
• Ability to take on-call shifts as needed

Demonstrates expertise in delivering patient-centered care for home dialysis, including training and support for patients and caregivers. Proficient in clinical assessment, care plan development, and compliance with healthcare regulations.
