Join CVS Health as a Case Manager, Registered Nurse in the field, where you will play a crucial role in serving dual eligible populations by addressing their complex health and social challenges through compassionate care and effective communication.

• Pay: $66,575.00 - $
• Location: Illinois, IL

• Schedule: Full-time
• Travel: Up to 50% travel in Kane, LaSalle and surrounding counties

• Facilitate the delivery of appropriate benefits and healthcare information.
• Develop, implement, and support health strategies and policies.
• Act as a liaison with members, employers, providers, and insurance companies.
• Implement and coordinate case management activities for catastrophic cases.
• Assess and analyze medical and vocational status of members.
• Communicate with members and stakeholders telephonically or in person.
• Prepare documentation of case work activities.
• Consult with internal multidisciplinary teams to maximize health outcomes.
• Provide educational and prevention information for best medical outcomes.
• Monitor member progress toward desired outcomes through assessment and evaluation.

• Associate’s Degree in Nursing (REQUIRED)
• Bachelor’s Degree in Nursing (PREFERRED)

• 3+ years clinical practical experience
• Preferred: 2+ years case management, discharge planning, and/or home health care coordination experience

• Active and unrestricted Registered Nurse (RN) in Illinois required
