The Minimum Data Set (MDS) Coordinator is responsible for ensuring that resident assessments, care plans, and interdisciplinary team meetings are completed accurately and in compliance with all Federal and State regulations. This role involves collaboration with physicians, nursing staff, residents, families, and other departments to optimize individualized nursing care within a Skilled Nursing Facility (SNF) setting.

Responsibilities:

Coordinate and complete resident assessment tools (MDS) in compliance with regulatory requirements.
Develop and update resident care plans based on assessment data and interdisciplinary input.
Facilitate and participate in interdisciplinary team meetings to discuss resident care and progress.
Collaborate with physicians, nursing staff, residents, families, and other facility departments.
Ensure documentation meets all Federal and State regulations and facility policies.
Monitor and maintain compliance with regulatory standards related to resident assessments.

Required Skills &

Certifications:

Active Registered Nurse license in the state of employment.
Minimum of 2 years nursing experience in hospital, home health, or hospice care.
Strong clinical assessment, medication administration, and treatment planning skills.
Excellent communication and interpersonal skills.
Ability to collaborate effectively with interdisciplinary teams.
Bachelor's degree in Nursing or related field.
Driver's license, car insurance, and automobile for field work.
