The Home Health Registered Nurse (RN) with OASIS experience is responsible for providing skilled nursing care in outpatient and home health settings. This role involves conducting patient assessments, managing care plans, and delivering hospice care to patients in their homes. The RN will travel between patient locations, utilize EMR systems for documentation, and collaborate with interdisciplinary teams to ensure quality patient outcomes.
Strong adaptability, communication, and a valid driver's license are essential for success in this dynamic role.
- Perform comprehensive patient assessments using OASIS (Outcome and Assessment Information Set) protocols.
- Develop, implement, and adjust individualized care plans for home health and hospice patients.
- Administer medications and treatments as prescribed and monitor patient responses.
- Coordinate care with interdisciplinary teams to optimize patient outcomes.
- Document patient care and clinical data accurately using the Health Wyse (Careficient) EMR system.
- Travel between patient homes to provide direct nursing care and support.
- Respond to changes in patient condition and adjust care plans accordingly.
- Maintain compliance with regulatory and organizational policies and procedures.
Certifications:
- Active Registered Nurse license in the state of Vermont.
- Minimum 2 years of experience in home health and hospice nursing.
- Proficient in OASIS assessment and documentation.
- Current BLS certification.
- Valid driver's license in good standing with reliable transportation.
- Experience with EMR systems, preferably Health Wyse (Careficient).
- Strong clinical assessment, medication administration, and treatment planning skills.
- Excellent communication and interpersonal skills.
Certifications:
- Previous experience in outpatient care settings.
- Additional certifications related to hospice or palliative care.
- Familiarity with other home health care documentation systems.
- Some on-call duties may be required on an emergency basis.
- Work involves traveling to multiple patient homes within the service area.
- Must be able to adapt to schedule changes and manage time effectively.
- Day shift schedule:
Friday, Saturday, Sunday, and Monday from 8:00 AM to 6:00 PM. - Shifts are fixed on specified days with potential emergency call requirements.
