Role Description

This position specializes in evaluating initial intake applications for individuals requesting supports and waivered services through the Division of Developmental Disabilities. A person in this role ensures that individuals meet all programmatic eligibility criteria, arranges comprehensive assessments, and provides a summary of findings and recommendations to a multi-professional team. This position plays an essential role to improve the quality of life, assure health and safety, support access to community participation, and increase opportunities for individuals with developmental disabilities and their families.

What

You’ll Do
  • Accept referrals for services from individuals, family members, agencies, and other interested parties.
  • Communicate with families about obtaining services by explaining services and eligibility, to assist in completing the application materials. Screen individuals to see if DMH-DD is the most appropriate service agency to meet their needs.
  • Reviews application for services.
  • Gather review and evaluate collateral information such as medical, educational and psychological records from a variety of sources in a timely fashion; identifies and correctly codes diagnoses.
  • Administers, scores, and interprets assessment tools including but not limited to the Missouri Adaptive Ability Scale (MAAS), Vineland Adaptive Behavior Scales (II).
  • Coordinate the initial MAAS with the Division Assessment Team.
  • Determine initial eligibility for persons applying for services based upon specific guidelines as set forth in State regulations.
  • Accurately and timely complete required documentation of eligibility/ineligibility findings and required notifications to applicants according to specific timelines.
  • Enter individual information into an electronic case record.
