Case Manager; RN, MSW, or RT

Job Overview

The Case Management team initiates discharge planning with every patient upon admission and provides assistance with aftercare, medical equipment, care coordination, and other discharge needs. Case managers are assigned to individual units but may also follow patients with specific discharge needs to provide targeted, patient-centered care. The team collaborates with physicians, nurses, ancillary staff, patients, and families to coordinate excellent care.

• Functions as a member of the multidisciplinary health care team to assess and plan for patients to leave the Health System.
• Completes psychosocial assessments for patients.
• Coordinates discharge plans, needs, and arrangements for patients.
• Provides information, education, and linkage with appropriate resources and services.
• Initiates discharge planning upon admission and assists with aftercare, medical equipment, and care coordination.

• Current Missouri Registered Nurse license or current Registered Nurse license from a compact state (must be in the nurse’s state of residence).
• Master of Social Work (MSW) degree.
• Licensed Respiratory Care Practitioner (RCP).
