Unit Description
Unit 5300 Acute Care Medicine at Barnes-Jewish Hospital (South Campus). 25‑bed unit.
Additional Information About the RoleNurses in Acute Care Medicine manage patients with a full range of clinical issues, from significant blood dyscrasias, advanced endocrine pathology, heart and renal failure, GI conditions, infectious diseases, asthma, and other conditions. The unit is led by a clinical manager and charge nurse, supported by an advanced practice nurse, case manager, social worker, physical therapist, dietitian, and pharmacist. The multidisciplinary team works with physicians, nurses and patient care technicians to provide quality care.
RolePurpose
Provides direct patient care activities including assessment, diagnosis, planning, implementation, and evaluation within the guidelines of the standards of nursing care.
Responsibilities- Promotes patient and family centered care in a healing environment; educates patients and families on illness management, home care needs, and medication administration.
- Participates in activities that promote patient safety, quality and regulatory compliance; engages in professional development.
- Develops, implements and documents individualized plans of care with defined goals in collaboration with the interprofessional team and family; proactively plans and ensures communication of care plans across the continuum of care.
- Uses critical nursing skills to assess and evaluate physical, psychosocial, and emotional needs according to standards of care; considers patient preferences, barriers, values, emotional, spiritual, cultural and population‑specific needs.
- Works in a constant state of alertness and maintains a safe environment as a safety‑sensitive position.
