Role
RN Care Manager II - IPCM
, part of an interdisciplinary team, assesses, plans, advocates, and coordinates patient care from admission to discharge, ensuring safe transitions and optimal health outcomes.
- Facilitate discharge planning.
- Develop individualized care plans.
- Coordinate services.
- Communicate effectively with patients, families, and healthcare providers.
- Intervene with patients with complex needs.
- Promote health education and seamless care transitions in alignment with quality, safety, and financial goals.
- Bachelor's Degree in Nursing.
- Current RN license.
- 1+ year of acute care hospital experience.
- BLS certification.
- Preferred certifications:
Case Management certification, with additional experience in care or utilization management considered advantageous.
- Clinical expertise.
- Critical thinking.
- Compassion.
- Excellent communication.
- Advocacy skills.
