Mercy South Hospice is seeking an Intake RN to coordinate Hospice, Home Health and Home Infusion referrals in acute care and clinic settings. The role includes supporting discharge planning, patient education, and coordinating with physicians and social workers in Mercy’s network.
The position offers a weekend rotation, 40 hours per week, and a range of benefits including health insurance, PTO, and retirement contributions. RN licensure in the applicable state is required;
Bachelor’s preferred.
