Care Manager

Mercy is seeking a PRN/As Needed Care Manager to coordinate patient care across the continuum, ensuring safe transitions, appropriate resource use, and strong patient advocacy. In this flexible, as-needed role, you will conduct clinical and psychosocial assessments, develop individualized care plans, and collaborate with physicians, nurses, social workers, and community partners to optimize outcomes. You will educate patients and families, arrange post-acute services, address barriers to care, and support complex discharge planning while documenting thoroughly in the EMR and upholding Mercy's compassionate, patient-centered values.

Responsibilities

• Conduct comprehensive clinical and psychosocial assessments for assigned patients.
• Develop and implement individualized care and discharge plans aligned with patient goals.
• Coordinate transitions of care, including post-acute referrals and community services.
• Collaborate with physicians, nurses, therapists, and social workers to optimize patient outcomes.
• Perform utilization review and ensure appropriate use of hospital and community resources.
• Educate patients and families on care plans, medications, and follow-up needs.
• Identify and address barriers to care, including social determinants impacting recovery.
• Document all assessments, interventions, and plans accurately in the EMR.
• Advocate for patients' needs while supporting Mercy's compassionate care standards.
• Provide PRN/as-needed coverage, adjusting to fluctuating census and acuity demands.
