Summary Of Care Manager RoleThe Care Manager collaborates within an interdisciplinary team to assess, plan, advocate, and coordinate patient care from admission to discharge, ensuring a safe and smooth transition post-hospitalization.
They focus on effective transition planning, monitoring length of stay, and maintaining patient safety, experience, and quality of care by utilizing performance metrics and best practices.
Qualifications Include- Graduate of an accredited School of Nursing
- Bachelor's Degree in Nursing
- Current state license
- BLS (CPR) certification at hire or within 90 days
- Preferred:
Case Management certification and care/utilization management experience
- Assess and coordinate patient care from admission to discharge
- Advocate for patient needs and safe transitions
- Ensure compliance with standards and improve care outcomes
- Collaborate with healthcare team to optimize patient experience and safety
