As a Case Manager at LIV Recovery, you help turn mission into measurable impact. You support people navigating complex systems, reconnecting with resources, strengthening stability, and moving toward personally defined goals. Your documentation and coordination work ensure services are responsive, accountable, and grounded in hope.

We are seeking a Case Manager who sees this role as mission delivery in action. You will walk alongside clients, listen to what matters, support individualized goals, and ensure services meet their voice, choice, culture, strengths, and goals.

What You’ll Do
  • Build trusting, respectful relationships with clients through consistent, strengths-based, person-centered engagement.
  • Partner with clients to identify goals, needs, preferences, strengths, supports, cultural considerations, and barriers.
  • Develop and follow individualized service plans that support recovery, wellness, housing stability, employment, education, independent living skills, and community connection.
  • Connect clients with treatment services, recovery supports, housing resources, mental health services, peer support, vocational resources, financial assistance, and other community-based supports.
  • Provide regular one-on-one case management support, monitor progress, document outcomes, and adjust plans with clients and care team.
  • Support clients in strengthening independent living skills, understanding tenant rights, budgeting, preventing relapse, and maintaining long-term housing stability.
  • Coordinate with clinical staff, supervisors, housing providers, community partners, and other stakeholders for timely access and continuity of care.
  • Maintain accurate, timely, person-centered documentation of assessments, service plans, progress notes, referrals, care coordination, follow-up, transition planning, and outcomes.
  • Support CARF and CSTAR expectations through documentation integrity, chart review participation, quality improvement, service coordination, and mission-aligned delivery.
  • Participate in multidisciplinary team meetings, case conferences, staff training, audits, corrective action follow-up, and continuous improvement.
  • Promote client rights, confidentiality, informed choice, self-determination, dignity, and meaningful participation.
  • Represent LIV Recovery’s mission, vision, and values via professional communication, ethical practice, accountability, and consistent attendance.
What You Bring
  • Bachelor’s degree in social work or a related field preferred; four years of experience in mental health, substance use, case management, housing services, or related field may be considered in lieu of a degree.
  • At least two years of experience with individuals experiencing behavioral health needs, substance use disorders, mental health disorders, homelessness, or related barriers.
  • Valid driver’s license required.
  • Strong verbal and written communication skills.
  • Strong computer skills – accurate data entry, documentation, information retrieval.
  • Ability to engage clients with empathy, professionalism, cultural humility, and respect for voice, choice, lived experience.
  • Ability to collaborate effectively with clients, families, team members, community partners, housing providers, and other service providers.
  • Knowledge of or willingness to learn CARF accreditation standards, Missouri CSTAR program expectations, behavioral health documentation requirements, client rights, confidentiality practices, and quality improvement processes.
  • Commitment to person-centered, recovery-focused, mission-driven service delivery.
Work Environment and

Physical Requirements
  • Work may occur in diverse environmental settings.
  • Some travel between locations is required.
  • Must be able to sit and stand for extended periods of time.
  • Must lift 25 pounds or more.
  • Must maintain regular and consistent attendance.
