Your Role as MDS CoordinatorAs our MDS Coordinator, you’ll play a critical role in ensuring accurate assessments, compliant documentation, and coordinated care planning from admission through discharge. Working closely with the Director of Nursing and the interdisciplinary team, you’ll help ensure our clinical and reimbursement processes reflect the true needs and status of our residents.
This is a key leadership position where your knowledge and organization directly impact care quality, survey outcomes, and facility success.
What You’ll Do- Collaborate with the interdisciplinary team to assess resident needs and coordinate MDS assessment schedules and care plan reviews
- Ensure decisions regarding completion of additional MDS assessments follow clinical criteria outlined in the RAI User’s Manual
- Assist in coordinating daily standup meetings, including review of resident care and setting assessment reference dates
- Ensure compliance with all federal, state, and certification requirements related to the RAI and MDS process
- Monitor MDS and care plan documentation to ensure accurate and supportive clinical records
- Maintain uptodate MDS status for assigned residents per regulatory guidelines
- Complete timely and accurate MDS coding through chart review, observation, and interviews with residents, families, and staff
- Enter resident data accurately into computerized MDS systems
- Participate in interdisciplinary team meetings, quality assurance activities, and audits
- Assist the DON with identification of significant changes, physician orders, and documentation updates
- Support scheduling and communication related to care plan meetings and outcomes
- Assist with clinical education or information systems operations as directed
- Unencumbered Kansas RN license or multistate license (required)
- Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred
- Strong understanding of:
- MDS 3.0
- OBRA regulations
- Medicare PDPM
- Medicaid reimbursement
- Case Mix and Quality Indicator processes
- Knowledge of the care planning process and interdisciplinary coordination
- Exceptional organizational, communication, and analytical skills
- Ability to work collaboratively while managing detailed, time sensitive responsibilities
