• Conduct initial and comprehensive resident assessments using the Minimum Data Set (MDS) and develop individualized care plans

• Lead and participate in care plan conferences to ensure comprehensive care

• Collaborate with the interdisciplinary team to develop and implement resident care plans

• Identify and prioritize resident needs, and coordinate care to meet those needs

• Maintain accurate and timely records of resident assessments, care plans, and progress

• Participate in quality improvement initiatives to enhance resident outcomes and care

Requirements:

• Current licensure as a Registered Nurse in the State of Kansas or Compact Licensure

• Minimum 2 years of experience in gerontological nursing or a related field

• Strong clinical skills and knowledge of gerontological nursing practices

• Excellent communication, organizational, and interpersonal skills

• Ability to work autonomously and as part of a team

• Strong attention to detail and ability to prioritize tasks effectively

• A competitive salary range of $87,000 - 93,000 annually
