Requirements:

• Active, full, unrestricted Alaska RN license – REQUIRED at time of submission
• Graduate of an accredited school of nursing
• High school diploma or equivalent
• Minimum 2 years of RECENT case management experience (within last 4 years)
• At least:
  • 1 year general nursing + 6 months outpatient clinical experience
    OR
  • 2 years general nursing with 1 year ambulatory experience
    OR
  • 1 year case management experience
• Ability to work in a fast-paced outpatient clinic setting
• Strong care coordination and interdisciplinary collaboration skills
• Proof of flu vaccination required
