Schedule:

Full-time outpatient clinic setting with multi-disciplinary team collaboration
Highlights:
Active case management role coordinating care for acute, chronic, and high-risk patient populations
Opportunity to work across multiple specialty areas including OB case management and specialty clinic coordination
Collaborate with interdisciplinary teams and providers to maximize patient health outcomes
13-week contract assignment in a community-focused healthcare environment
Utilize electronic health records to manage patient panels and track care coordination
Participate in staff nursing duties including assessment, patient education, and clinical support
Requirements:
Current, unrestricted Alaska RN license required at time of application
Minimum 2 years of recent case management experience within the last 4 years
Graduate of an accredited school of nursing;
Bachelor's degree preferred
At least 1 year general nursing experience with 6 months in outpatient clinic setting, OR 2 years general nursing with 1 year ambulatory or case management experience
Basic Life Support certification preferred
National Case Management certification preferred
Proof of flu vaccine required
Fingerprint clearance required; allow minimum 5 weeks processing time before start date
Must reside 50+ miles from facility location
