Requirements:
Active RN license in the practicing state.
Experience in case management, utilization review, or clinical coordination preferred.
Strong organizational, clinical, and communication skills.
BLS required; CCM or specialty case management certification preferred.
Responsibilities:
Assess patient needs and develop individualized care plans.
Coordinate care across multidisciplinary teams and healthcare settings.
Monitor patient progress and adjust care plans as needed.
Educate patients and families on treatment plans and available resources.
Ensure compliance with regulatory standards, documentation, and quality metrics.
