Description:
This position functions within the nursing department.
It is responsible for coordinating the delivery of appropriate and timely care for patients.
The role promotes quality and cost-effective health care outcomes.
The individual will manage patient needs actively.
Collaboration with medical providers and interdisciplinary team members will be essential.
Establish individualized medical goals to maximize patient healthcare outcomes.
Coordinate multi-disciplinary care conferences for high-risk or complex patients.
Review electronic health records and provide outreach and education to patients.
Participate in case management reviews with a multi-disciplinary team.
Advocate for patients and families, fostering independence and self-care.
Use problem-solving skills to meet the needs of patients efficiently.
Facilitate the coordination of patient referrals and track their progress.
Serve as the primary link for case managers and discharge coordinators.
Address delays and obstacles to care proactively.
Perform nursing duties including assessment, documentation, and medication administration.
Maintain annual competencies including safety and infection control.
Participate in department and clinic team improvement efforts.
Respond to emergency situations as required in a clinical role.
Facilitate specialty clinic referrals and manage waitlists.
Ensure appropriate diagnostics are ordered and completed prior to appointments.
Track referral progress and provide status reports to providers.
Assist with educational in-services provided by specialty clinic staff.
Manage intake and tracking processes for patients.
Communicate and coordinate video teleconferencing visits with specialty providers.
Support referral management for specific tests and coordinate appointments.
Maintain a clean and safe working environment.
Adhere to infection control policies and procedures.
Perform other duties as assigned.
Develop good working relations with staff and departments.
Ability to multi-task and work independently in a fast-paced environment is essential.
Must be able to lift and move 50 lbs.
