Requirements:
Active Registered Nurse (RN) license
Diabetes education or case management experience preferred
Certified Diabetes Care and Education Specialist (CDCES) preferred or willingness to obtain
BLS certification required
Strong patient education and care coordination skills
Excellent communication and organizational abilities
Responsibilities:
Provide individualized diabetes education and self-management training
Assess patients’ clinical needs and develop care plans
Monitor blood glucose trends and treatment adherence
Coordinate care with providers and interdisciplinary teams
Educate patients on medications, insulin administration, nutrition, and lifestyle changes
Document patient progress and outcomes accurately
