Job Description:

Responsible for the active management of patients, collaborating with medical providers and interdisciplinary team members to establish individualized medical goals maximizing patient healthcare outcomes.
Responsibilities:
Collaborate with medical providers and interdisciplinary team members.
Coordinate multi-disciplinary care conferences and manage care needs.
Facilitate patient referrals and serve as the primary link for case managers and discharge coordinators.
Maintain safety, infection control requirements, and a clean working environment.
Requirements:
MUST HAVE ACTIVE ALASKA LICENSE at time of submittal.
At least 2 years of recent case management experience (within the last 4 years).
Graduate of an accredited school of nursing;
Bachelor's degree preferred.
Current full, unrestricted Registered Nursing license in Alaska required.
