Hospital Level Care at Home Intake Nurse – Intermountain Health, Juneau, AK.
Job DescriptionThe Nurse Case Manager utilizes clinical expertise and critical thinking skills to develop and implement a plan of care that provides cost‑effective, extraordinary care. This position works collaboratively with patients, patient families/significant others, healthcare providers, insurers, community resources, and other involved parties.
Job Essentials- Discuss and explain the HLC@H program with providers and hospital staff.
- Discuss the Hospital Level of Care @ Home (HLC@H) program with patients and families.
- Onboard patients being transferred to the HLC@H program.
- Organize and prioritize daily work by assessing new, current, and discharging patient needs in areas of responsibility. Complete documentation as required.
- Screen and coordinate the discharge planning process from patient admission to departure. Assess patient needs, develop a discharge plan, and coordinate with internal and external services for care during and after the hospital stay.
- Identify patients with potential avoidable delays, extended stay, readmission, or discharge needs. Coordinate with care providers and leaders to reach optimal solutions.
- Educate, communicate, and coordinate with multiple patient care teams to ensure the patient receives quality care, a timely discharge, and to identify and resolve delays and issues.
- Set up the RPM‑T kit and walk patients through how to use equipment.
- Communicate between providers, Tele RN staff, and Pharmacy for patient care.
- Variable hours and some weekend shifts will be required (differentials available).
- This role is onsite (
not remote
). - The work location for this position is Logan Regional Hospital.
- Current RN license in the state where the nurse practices.
- Three years of clinical nursing experience.
- RNs hired or promoted into this role must have or obtain their BSN within three years of hire or promotion.
- Bachelor's degree in Nursing (BSN) from an accredited institution.
- Case management Certification.
- Experience in case management, utilization review, and/or discharge planning.
- Ongoing need to see and read information, labels, monitors, identify equipment and supplies, and assess patient needs.
- Frequent interactions with patient care providers, patients, and visitors that require verbal communication, hearing, and understanding of spoken information, alarms, needs, and issues quickly and accurately, especially during emergency situations.
- Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy, including frequent computer use and typing for documenting patient care.
Scheduled weekly hours: 40. Hourly rate: $44.42 – $65.70, dependent upon experience.
