Summary

This position is eligible for the Education Debt Reduction Program (EDRP) – a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program approval – award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application.

Former EDRP participants are ineligible to apply.

Responsibilities
  • Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence‑based practice.
  • Influences and models the delivery of evidence‑based health care – demonstrating expertise in managing a patient panel, population, or program.
  • Develops and implements an individualized plan to achieve optimal population health and disease management.
  • Demonstrates leadership and works collaboratively with the Primary Care Team and other interdisciplinary team members.
  • Orders and interprets diagnostic studies.
  • Prescribes pharmacologic and non‑pharmacologic interventions.
  • Initiates referrals and consults to specialty services and provides education to patients and their families.
  • Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes.
  • Leads and implements changes that enhance the quality and timeliness of Veteran/patient care, applying research and evidence in the delivery of health care for patient populations.
  • Initiates and leads interdisciplinary teams informed by structured approaches and processes to improve outcomes that positively impact care for a patient panel, population, or program.
  • Acquires knowledge and skills, then disseminates acquired information within and/or outside the medical facility.
  • Initiates and leads interdisciplinary teams developing systems that positively impact outcomes at the panel, population, or program level.
  • Demonstrates effective communication skills and professional behavior – promoting cooperation and teamwork.
  • Exemplifies the ICARE core values of Integrity, Commitment, Advocacy, Respect, and Excellence.
  • Manages chronic disease conditions including hypertension, diabetes, ischemic heart disease, heart failure, chronic pain, and chronic obstructive pulmonary disease using clinical practice guidelines.
  • Acts as a surrogate for patient colleagues when one or more of their colleagues are on leave.
  • Documents and completes administrative work in a timely manner.
  • Addresses clinical reminders as medically appropriate.
  • Maintains a quality of care meeting professional standards.
  • Participates in activities, committees, boards, or projects as dictated by strategic or other initiatives/taskings.
  • Supplies supervision or collaborates with a physician for a mid‑level provider.
  • Provides clinical oversight and review.
  • Has supervised or collaborated responsibilities noted in the physician’s performance evaluation.
  • Manages a panel of patients.
  • Adheres to departmental and facility policies for leave and attendance and works within appointment guidelines.
  • Provides preventive health for both male and female patients.
Qualifications
  • 1 year of primary care experience preferred.
  • Skill in interpreting EKGs preferred.
  • Knowledge of or willingness to learn and incorporate virtual care visits.
  • License: one full and unrestricted license from any U.S. State or territory.
